Hospice of Charleston affirms that the most important members of the team are the patient and family. Our team wants to make the patient and family the center of our care circle . We provide compassionate care through a holistic team approach focusing on physical, emotional, social and spiritual needs.

Our nursing staff consists of Registered Nurses who will work together with patients and families as a team to formulate the plan of treatment. This is a written care plan which identifies specific goals and techniques to accomplish these goals. There is always a registered nurse available on call 24 hours a day. Please discuss any problems or concerns with them. The Registered Nurse is the case manager and every patient is evaluated with the medical director once every two weeks.

Hospice of Charleston is also staffed with Certified Nursing Assistants known as Hospice Aides. They are highly valued and important members of the health care team. They assist with administering personal care under the direction of the nursing staff.

Social Workers are available to identify options and resources as well as coordinate and provide counseling services for patients and the family. Please notify the social worker if you wish to discuss any areas of concerns. Social Work staff is also available on call 24 hours a day and are mastered prepared. They help identify resources for the patient to ease concerns during this difficult time.

Volunteers are the heart of hospice care. Volunteers are able to provide some social support to patients and families. We also have a select team of volunteers that are available in the Hospice Inpatient Center to provide a variety of services for patients. We have over 100 volunteers who can run errands, provide respite time for the caregiver, and help in time of crisis.

Hospice Chaplains visit patients and families for support and are available to discuss spiritual issues and end of life care for loved ones. Our Chaplains work in partnership with local clergy as well as helping families connect with representatives of their faith traditions in the Greater Charleston area. Spiritual support in hospice does not promote any one-belief system, nor do the chaplains seek to change the belief system of patients or families. The chaplains seek to be loving and supportive presences during this part of the spiritual journeys of the patients and families. The chaplains are also mastered prepared. They assist the patient, the families and staff.

The Hospice Inpatient Center includes the Peace Chapel that is available for quiet prayer and meditation as well as for memorial services for any patient. The center has 20 beds available. Patients go there for symptom management and respite, both of which the patient can return home and for end of life care. The center is located at 676 Wando Park Blvd. in Mt. Pleasant. Hospice of Charleston's hospice home care and inpatient center are driven under the direction of our medical directors. The medical directors are there to insure that medications are properly managed and to consult with the patient's primary care physician if any medical changes or palliative care interventions are necessary. Some of the medical directors are board certified in Hospice and Palliative Medicine. They make rounds at the Center to direct the patient's medical care.
